I(we) verify that I am (we are) legally eligible to give the consent contained within this release section. I (we) do hereby give permission for any minors named on this form to travel on this mission trip and release all liability on their behalf as well.

In the event of a medical emergency, I (we) hereby consent to the necessary and proper treatment, surgery, and/or anesthetic by a licensed physician or health care professional for any individual(s) named on this form.

I am (we are) aware of the potential risks to persons or property and self while participating in the Reel Life International, Inc. (RLI) program. With such knowledge, I (we) voluntarily release RLI, its associates and representatives, as well as the host organization and their representatives and employees, from any and all liability related to the activities of this program. By checking the box "I (we) Agree" below, I understand that I am giving this consent to this release of liability.

I understand that my family members and I will be required to abide by the guidelines of Reel Life International, Inc. (RLI), our Team Leader and the local host while on this Mission Trip. I understand that any failure to abide by these guidelines may result in being required to withdraw from this program at my expense. By checking this box, I hereby agree to abide by them.
